Two month supply of Semaglutide/Tirzepatide (delivered to your home) including all supplies

Access to the patient portal to speak with your provider with for ongoing communication

SUBSEQUENT WEIGHT LOSS VISITS: $75 ESTABLISHED PATIENT
30 minute Telehealth visit with a Board Certified Medical Provider

During these visits we will review progress and goals.

Re-ordering of Semaglutide/Tirzepatide

Implementing new goals and treatment plan

Discuss staying at current dose, switching medications or increasing the dose (remember if you are losing weight there is no need to increase dose)

Discuss repeating future labs work to determine improvement   

We do not prescribe GLP-1 weight loss medications without conducting a proper assessment. We believe in taking the time to get to know our patients by completing a thorough assessment along with specific lab panels to monitor your improvement along the way GLP-1 vials of medication starting off as low as $165 per vial.

Please note that dosage adjustment is individualized and if weight loss is ongoing there’s no need to increase the dosage

Insurance In-Network

I accept the following insurances: Anthem Blue Cross Blue Shield, Aetna, Amerigroup and Beacon Health.

Insurance Out of Network

If you have health insurance with a provider not listed, you may be able to use out-of-network benefits, in which your insurance provider may reimburse you a portion of monies spent on services.  Overall, as your plan varies, your provider will give you specific details about how much they reimburse, if any, for mental health or behavioral services.

Payment

All fees for services, including co-payments, are due 24 hours prior to your appointment by utilizing the patient portal.

Cancellation Policy

If you are unable to attend a session, please provide a 24-hour notice to avoid a late cancellation fee. If you do not provide a notice, you will be charged a $35.00 missed appointment fee. If you have two missed appointments without advanced notice, no further sessions will be scheduled at that time.

Good Faith Estimate

Effective January 1, 2023, you are entitled to receive a “Good Faith Estimate” of what charges could be for psychotherapy services provided to you.  While it is not possible for a psychotherapist to know in advance how many psychotherapy sessions may be necessary or appropriate for a given person, this form provides an estimate of the cost of services provided.  Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you.  This estimate is not a contract and does not obligate you to obtain any services from the provider.

The Good Faith Estimate is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits.  The number of visits that are appropriate in your case, and the estimated cost for those services, depend on your needs and what you agree to in consultation with your therapist.  You are entitled to disagree with any recommendations made to you concerning your treatment, and you may discontinue treatment at any time.

If you are paying for services privately or out of pocket, a Good Faith Estimate of the costs you may incur for psychotherapy services will be provided to you prior to your initial appointment.
